1. Professional Disclosure Statement
At Optimum Behavioral Health & Wellness, LLC, we want you to fully understand your rights when receiving care. Each of our social workers meets the education, training, and experience standards required by Louisiana law. You may review public records maintained by the Louisiana Board of Social Work Examiners (LABSWE) that verify your provider’s credentials, and you may also request a copy of the LABSWE Standards of Practice at any time. Before beginning services, you will be informed of the fees associated with your care, and you have the right to privacy and confidentiality as permitted by law. 2. Informed Consent for Services
At Optimum Behavioral Health & Wellness, LLC, your treatment is based on informed consent, which means that you have the right to fully understand and agree to the services being provided. Before you begin, your provider will explain your treatment plan, goals, and alternatives so that you may make the best decision for your care. We will review the costs of services before they begin, and you have the right to refuse or stop treatment at any time without penalty. Consent will apply for the duration of your care unless you choose to withdraw it. You will always have the opportunity to ask questions and receive clear answers about your care.
3. Confidentiality and Its Limits
Your privacy is a priority at Optimum Behavioral Health & Wellness, LLC. Information you share with your social worker will remain confidential, with certain exceptions required by law. These include mandatory reporting of suspected child, elder, or vulnerable adult abuse or neglect, situations in which you pose a serious threat of harm to yourself or others, and court orders or subpoenas that legally require disclosure. You may also choose to provide written consent for information to be shared with a third party, such as for billing or care coordination. No sessions will ever be recorded, nor will a third party be allowed to observe, without your written consent.
4. LABSWE Standards of Practice (Summary)
At Optimum Behavioral Health & Wellness, LLC, our social workers follow the Standards of Practice set by the Louisiana Board of Social Work Examiners (LABSWE). These standards ensure that every client is treated with professionalism, respect, and care. Social workers are required to maintain good moral character, promote client welfare, respect each client’s right to make their own choices, and provide services without discrimination. They must also ensure that clients understand their treatment plans, diagnostic results, and any alternatives, while regularly reviewing and updating care as needed. Social workers are expected to seek supervision or consultation when necessary and to maintain accurate, complete records for the required length of time. Informed consent must always be obtained before beginning services, and treatment must be ended responsibly, with appropriate notice and referrals when needed.
